1736 Family Crisis Center offers crisis intervention services, including four 24- hour hotlines, as well as longer-term services in client advocacy and outpatient psychotherapy. The residential counseling and support services include shelter programs for (1) runaway and homeless adolescents, (2) domestic violence and human trafficking victims and their children (emergency and transitional programs (3) Homeless prevention and rapid re-housing services for the general population and Veterans and their families. Community outreach programs are provided to educate the public and prevent mental-health problems in the general population and in targeted groups to at-risk populations.
1736 Family Crisis Center, Community Resource Center is seeking a Temporary Case Manager (up to 15 months) to oversee case management services for survivors of domestic violence. This role is responsible for ensuring that the program meets performance outcomes, overseeing reporting requirements, and providing leadership to the case management team. The ideal candidate will have experience in case management, program compliance, and staff supervision, with a strong commitment to trauma-informed, survivor-centered care.
Key Responsibilities:
• Conduct comprehensive client assessments to determine individual needs and develop case plans.
• Provide 1-on-1 case management, ensuring clients have access to counseling, medical care, job development, and other vital services.
• Facilitate individual, group, and family support sessions, as needed.
• Deliver life skills training, including resume building, interview preparation, budgeting, stress management, and communication skills.
• Assist clients with job development and career planning.
• Develop and maintain strong community partnerships to provide appropriate client referrals.
• Accompany clients to court hearings and social service appointments, offering advocacy and support.
• Assists with client therapeutic recreation/art activities
• Provides for the physical safety of clients in the event of accidents or disasters; Helps
• Coordinate/conduct client fire and earthquake drills, as directed
• Answers telephone for business-related calls
• Provide crisis intervention and emotional support to clients during assigned shifts.
• Ensure adherence to program guidelines, house rules, and safety procedures.
• Maintain accurate and timely documentation of case notes and services provided.
• Reviews assigned cases in all aspects and takes responsibility for completion of all goals and objectives. This includes ensuring that all record keeping, advocacy, and social work services are carried out as needed, in a high quality and timely manner
• Provide direct case management services as needed, including referrals to housing, legal aid, counseling, and job development resources.
• Conduct client support groups, parenting education workshops, and therapeutic recreational activities.
• Participate in staff meetings and contribute to program outreach efforts.
Qualifications
• Bachelor’s degree in Social Work, Psychology, or a related field and 1 year experience in case management OR a high school diploma with at least two (2) year of case management experience working with vulnerable populations.
• Prior experience in case management, advocacy, or social services, preferably with survivors of domestic violence.
• Strong organizational skills and ability to manage multiple priorities in a fast-paced environment.
• Proficiency in Microsoft Word, Excel, and case management software.
• Excellent written and verbal communication skills.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Commitment to trauma-informed, client-centered, and culturally competent care.
Preferred Qualifications:
• Bilingual in Spanish is a plus but not required.
• Knowledge of local and state resources for domestic violence survivors.
