1736 Family Crisis Center offers crisis intervention services, including four 24- hour hotlines, as well as longer-term services in client advocacy and outpatient psychotherapy. The residential counseling and support services include shelter programs for (1) runaway and homeless adolescents, (2) domestic violence and human trafficking victims and their children (emergency and transitional programs (3) Homeless prevention and rapid re-housing services for the general population and Veterans and their families. Community outreach programs are provided to educate the public and prevent mental-health problems in the general population and in targeted groups to at-risk populations.
1736 Family Crisis Center, Community Resource Center is seeking a Temporary Lead Case Manager (up to 15 months) to oversee case management services for survivors of domestic violence. This role is responsible for ensuring that the program meets performance outcomes, overseeing reporting requirements, and providing leadership to the case management team. The ideal candidate will have experience in case management, program compliance, and staff supervision, with a strong commitment to trauma-informed, survivor-centered care.
Key Responsibilities:
• Ensure program compliance with performance metrics, funding requirements, and agency policies.
• Monitor and evaluate client progress, program goals, and service outcomes, including conducting client file reviews and quality assurance monitoring.
• Prepare and submit required reports, tracking key performance indicators and client data.
• Maintain accurate documentation of case management activities and client services.
• Conduct client assessments and oversee the development of individualized service plans.
• Assigns and reviews assigned cases in all aspects and takes responsibility for completion of all goals and objectives. This includes ensuring that all record keeping, advocacy, and social work services are carried out as needed, in a high quality and timely manner
• Provide direct case management services as needed, including referrals to housing, legal aid, counseling, and job development resources.
• Support crisis intervention efforts and ensure proper response protocols are followed.
• Facilitate and oversee life skills training, support groups, and advocacy services for clients.
• Accompany clients to court hearings and social service appointments when needed.
• Provide guidance and support to case managers, ensuring quality service delivery.
• Train and mentor staff on best practices in trauma-informed care, case management, and client advocacy.
• Lead case reviews and staff meetings to ensure consistency and effectiveness in service provision.
• Strengthen partnerships with community organizations to expand available resources for clients.
• Represent the program at community meetings, coalitions, and outreach events.
• Advocate for survivors’ needs within the social service and legal systems.
• Provides client crisis intervention during scheduled shift.
• Assists with client therapeutic recreation/art activities
• Provides for the physical safety of clients in the event of accidents or disasters; Helps
• Coordinate/conduct client fire and earthquake drills, as directed
• Answers telephone for business-related calls
Qualifications
• Bachelor’s degree in Social Work, Psychology, or a related field and 2 years of experience in case management OR a high school diploma with at least three (3) years of case management experience in domestic violence, homelessness, or related social services.
• Minimum 1-2 years of leadership or supervisory experience in a social service setting.
• Experience managing program performance metrics and reporting outcomes.
• Strong understanding of trauma-informed care, crisis intervention, and survivor advocacy.
• Excellent organizational, analytical, and problem-solving skills.
• Proficiency in Microsoft Office Suite and case management software.
• Ability to work collaboratively in a fast-paced environment while maintaining confidentiality and professionalism.
Preferred Qualifications:
• Bilingual in Spanish is a plus but not required.
• Knowledge of local and state resources for domestic violence survivors.
• Experience working with local grant-funded programs and compliance requirements.
