About Us:

We’re currently expanding and looking for a Community Liaison who’s passionate about building relationships and connecting families with the services they need.
Position Summary:
As our Home Health Community Liaison, you will be the face of 1st Americare in the community—connecting with hospitals, physician offices, rehabilitation centers, senior living facilities, and families. Your focus will be on identifying potential clients and referral sources while educating the community about our Medicare-certified home health services.
Key Responsibilities:
- Develop and maintain relationships with referral sources including hospitals, discharge planners, case managers, physicians, and social workers
- Represent 1st Americare at health fairs, community events, and networking meetings
- Educate referral sources and families on 1st Americare’s services and the Medicare home health benefit
- Identify and pursue new marketing and referral opportunities
- Track referrals, admissions, and outcomes to help measure success
- Collaborate with clinical and intake teams to ensure smooth onboarding of clients
Requirements:
- 2 years of experience in the home health industry—specifically working with Medicare-certified services
- Existing referral network or established relationships with local healthcare providers is a strong plus
- Knowledge of Medicare guidelines, eligibility, and home health regulations
- Excellent communication, interpersonal, and organizational skills
- Self-starter with a passion for helping others and improving patient outcomes
- Reliable transportation and ability to travel locally
