Building Location: 32nd Avenue Building Department: 3011240 PALLIATIVE CARE - 32ND HOSP Job Description: Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention. They may care for patients ranging in age from newborn to elderly as noted in age-related category and area of expertise. This is a unique opportunity to be part of a collaborative, interdisciplinary team dedicated to delivering holistic, patient-centered care that supports individuals and their families through some of life’s most meaningful moments. PRACTICE SPECIFICS Full-time; Mon-Fri Play a critical role in delivering comprehensive care to patients with serious illnesses. Deliver care within the inpatient setting. Close collaboration and support within the interdisciplinary palliative care team across the inpatient and outpatient settings which includes 1 Fellowship Trained Physician, 1 Social Worker, and 1 chaplain. Opportunity to participate in the ongoing growth and development of the palliative care program. EPIC (Electronic Medical Record) for both our Inpatient and Ambulatory facilities Competitive salary and comprehensive benefits package REQUIREMENTS ·Significant experience in palliative medicine, oncology, hospice or geriatrics is strongly preferred.
