Medi Home Health Agency, a proud member of the Medical Services of America Inc., seeks a Part-time / 32 Physical Therapy Assistant
for our Home Health patients in Fort Walton Beach, Navarre (South Santa Rosa, Okaloosa), FL.
- Provides physical therapy services to patients under appropriate guidelines and in accordance with the plan of treatment developed by the PT.
- Assists the physician in implementing and conducting treatment as outlined in treatment plan.
- Monitor patient's response to treatment and modify treatment as indicated in accordance with the plan of care and reports to the physician or client care manager.
- Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies.
- Assists with interpreting and communicating evaluation findings to patients, family and other staff members.
