Job Description
Medi Home Health Agency is seeking an experienced Part-Time/PRN Physical Therapist for our Home Health patients in Lock Haven, PA.
- Provide physical therapy services to patients under appropriate guidelines.
- Implement and conduct treatment as outlined in the treatment plan.
- Treat clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments and procedures within the plan of care.
- Monitor patient's response to treatment and modify treatment as indicated in accordance with physician's orders.
- Interpret and communicate evaluation findings to patients, family, and other staff members.
Responsibilities
Key Responsibilities:
- Conduct home health physical therapy evaluations to assess patient needs.
- Develop and implement individualized treatment plans that meet patient goals and objectives.
- Perform various physical therapy interventions to promote patient progress and recovery.
- Communicate effectively with patients, families, and healthcare team members to ensure quality care and coordination.
Requirements
Qualifications:
- Valid Physical Therapy license in the state of Pennsylvania.
- Valid Driver's License.
- Current CPR certification.
- Minimum of one year verifiable physical therapy experience.
- Home Health experience preferred.
