AGENCY DESCRIPTION:
360 Community Services is a Nebraska DHHS certified provider for specialized community-based programs and services with people with developmental disabilities, offering residential and day center placements, with services centered around individualized habilitation programs to increase the skills and independence of people with developmental disabilities. 360 Community Services' team consists of highly passionate professionals with a wealth of experience in matters of finance, medical protocols, staff development and training, behavior modification, crisis intervention, research-based habilitation and behavioral support planning, and client funding.
SUMMARY DESCRIPTION:
The Residential Coordinator provides case management to approximately 20-25 contracted Shared Living Providers whom serve individuals in a residential community setting. Residential Coordinators provide support to their assigned Shared Living Providers and serve as a liaison between an individual’s treatment team.
ESSENTIAL FUNCTIONS :
- Ensures supports & services provided to individuals on your caseload comply with NE DHHS regulations referenced in - TITLE 404 COMMUNITY-BASED SERVICES FOR INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES
- Provides ongoing supervision and support to Shared Living Providers through monthly home visits; attendance at annual and semi-annual ISP meetings; and available for 24-hour emergency supports via phone or in-home visits.
- Develops, tracks, and updates client specific habilitation & behavioral programming.
- Ensures client rights are maintained and respected via in-home visits, GER audits, and attendance at quarterly Human Right Committee meetings.
- Provides timely reporting/notification of Critical incidents to Director of Residential Services and 360 Administration.
- Collaborates with Director of Nursing to ensure client specific protocols, medications, or medical needs are in place and are adhered to by the Shared Living Provider.
- Collaborates with the Director Clinical/Assessment Services to ensure client safety plans, behavioral support plans, behavior trackers are person-centered, implemented, and required documentation completed. Additionally, assists Clinical Director in managing Risk funded clientele to ensure safety for both the individual and the community.
- Works with finance team to ensure Shared Living Providers conduct proper management of clients funds, completes attendance tracking, and provider general support in any billing matters.
- Collaborates with schools, health care providers, mental health professionals, and other community resources to ensure individual in care has community resources needed to encourage mental balance, community inclusion, and independence.
QUALIFIACTIONS/REQUIREMENTS:
Work Experience:
- Prior employment experience with case management is required.
Minimum age of 21 years
Valid Driver’s License
Current vehicle Insurance upon request
The incumbent in this position will be required to pass a State & Federal background check.
Education:
High school diploma/GED required (College degree in Education, Psychology, or Social Sciences preferred).- The following certifications are required -will provide training if necessary:
CPR/1st Aid
Medication Aide- Safe Crisis Management (SCM)
Physical Requirements:
- Possess the manual dexterity, physical ability, and stamina for a full day of various physical activity that may include maintaining student safety through physical interventions; as well as frequent walking, standing, stooping, lifting up to approximately 25 pounds and occasional lifting of equipment and/or materials weighing up to approximately 40 pounds may be required.
Must have the ability to see and read a computer screen and printed material with or without vision aids; hear and understand speech at normal levels and on the telephone; speak in audible tones so that others may understand clearly in person and on the telephone; and the ability to understand and follow oral and written instructions.
