We are providing residential and day program services to participants in Nebraska with intellectual and developmental disabilities.

We're looking for Residential Coordinator  to join our team! As a Residential Coordinator, you will provide support and guidance to people served and their Shared Living Provider as they learn and develop important daily living skills and life activities to live as independently as possible.

SUMMARY OF MAIN RESPONSIBILITIES

Residential Coordinator manages Shared Living Providers AND DPS to ensure the highest quality services are provided to individuals in that setting. Maintain monthly, in person meetings at the SLP to ensure individual is well cared for. Manage a caseload of individuals that have developmental disabilities and mental health issues. Develop and maintain programs and attend biannual meetings.

SPECIFIC RESPONSIBILITIES:

• Ensure compliance with agency policies and procedures, contractual agreements established with Nebraska’s Department of Health and Human Service System, as well as ensuring compliance with state, federal and local regulations and laws.
• Ensures client rights are maintained and respected via in-home/work visits, GER audits, and attendance at quarterly Human Right Committee meetings.
• Collaborates with Director of Operations (DO) and team to ensure client specific protocols, medications, or medical needs are in place and are adhered to by the Shared Living Provider and Day Staff.
• Collaborates with DO to ensure client safety plans, behavioral support plans, behavior trackers are person-centered, implemented, and required documentation completed.
• Quality Assurance of data collection and client records.

RESIDENTIAL SPECIFIC TASKS:

• Responsible for planning, implementing, coordinating, and evaluating all service provisions rendered through the Residential Department.
• Make sure our Shared Living Providers deliver the highest level of quality services possible, while maintaining compliance with the individual’s program plan. - Performing walk through, montly data etc..
• Complete monthly, in person audit of SLP residence to ensure fidelity of programming and safety of individual services. Includes additional training and remediation as needed.
• Provide 24-hour crisis intervention support to SLP via phone or in-home visits.
• Monthly audits and updates client specific habilitation & behavioral programming.
• Performing Monthly report, and others tasks assigned by the DO or CEO 
• Creating, adjust, modifying safety plan. 
• Completing monitoring activities and providing technical assistance to verify the accurate and timely completion of required documentation.
• Attending Individual Support Planning meetings
• Write quality programs on or before the ISP meetings/intakes. Development of programming by conducting behavioral and skill assessment of clients though behavioral assessments to identify and access individual strengths and needs.
• Review GERs within 24 hours of submission and communicate with Director of Operations when GER is ready to approve.
• Creating habilitative programs and activities for the individuals assigned to the coordinator's caseload.
• Works with administration to ensure Shared Living Providers conduct proper management of clients funds, completedattendance tracking, and provider general support in any billing matters.
• Function as a member of a treatment team for both adults and children in the system. This includes working with a Therapists, guardians, physicians, DHHS workers, and child and family services professionals. Work in collaboration with schools, providers, mental health professionals, and other community resources.

Education:

• Bachelor's (Preferred)

Experience:

• Case management: 1 year (Preferred)
• Crisis management: 1 year (Preferred)
• Program/Goal Writing: 1 year (Preferred)

License/Certification:

• CPR Certification (Preferred)
• Mandt System
