Position Summary:
The Team Lead for our Day Habilitation Community Integration Center will be an on-call position, Monday through Friday from 7:00 AM to 3:00 PM. This role is vital in ensuring that Direct Support Professionals (DSPs) adhere to agency policies and procedures while remaining in compliance with all ISP (Individual Service Plan) contract requirements. The Team Lead will be actively engaged in the field daily from 9:00 AM to 2:00 PM, supervising and supporting program staff to achieve high-quality outcomes for individuals served.
Key Responsibilities:

Supervision and Compliance:
- Monitor DSP activities in the community to ensure compliance with ISP requirements, agency policies, and professional conduct.
- Develop and maintain an internal quality compliance system to meet agency and regulatory standards.
- Train DSPs on agency policies, UI/MUI protocols, and ensure timely and accurate reporting.
- Program Oversight and Implementation:
- Develop, implement, and evaluate programs that align with participant goals and outcomes.
- Ensure DSPs are following scheduled activities and actively supporting participant integration into the community.
- Act as a backup for DSP and transportation duties when needed, requiring a valid driver's license.
- Relationship Management:
- Maintain professional relationships with individuals, their support groups, and external stakeholders.
- Facilitate communication and coordination of care between individuals, staff, families, and support teams.
- Address complaints, grievances, and appeals related to individual services promptly and effectively.
- Staff Development and Retention:
- Ensure all DSPs are trained and maintain required certifications, including Medication Administration and CPR.
- Implement retention programs to support staff morale and long-term engagement.
- Attend and coordinate on-site meetings to foster collaboration and team growth.
- Documentation and Administrative Duties:
- Maintain accurate and up-to-date records of monthly and yearly UI/MUI reports.
- Monitor program outcomes and adjust strategies to improve effectiveness.
- Ensure timely and accurate documentation of services provided.
Qualifications:
- Proven experience in program management and staff supervision, preferably in the DODD, behavioral health, or social work fields.
- Familiarity with DODD regulations, ISP requirements, and best practices in community integration.
- Strong organizational and administrative skills, including compliance oversight and reporting.
- Excellent interpersonal and communication skills to effectively engage with diverse populations, staff, and stakeholders.
- Ability to train and mentor staff to maintain high-quality service delivery.
- Valid driver’s license, and ability to obtain and maintain certifications in Medication Administration, CPR, and DODD training.
- Background check clearance required.

- Background in social work, behavioral health, or related fields.
