4 Sisters is a family-centered, trauma-integrated, innovation Case Management Company and we are expanding to our new location in Dixie, Levy, Gilchrist, and Taylor counties.

Role

This position is intended to support the full-service case management model’s success in meeting safety, permanency, and well-being goals are met during periods of transition which include expansion, full-time employee leave (medical, PTO, bereavement, vacancy) by completing case management activities such as home visits with children, face to face contact with parents, home studies, supervised visits, safety planning, etc. Assignments can be task focused, or caseload coverage as agreed to by the employee and supervisor.

Essential Job Functions

The essential functions identified are intended to highlight those assignments that are task specific as well as those when a caseload is temporarily covered.

  • Complete case management assignments with integrity, competency and adherence with statute, F.A.C, CFOP, and agency and system of care policy and procedure.
  • Document case management activities completed in FSFN within 48 hours of activity.
  • Documentation is fact-based, timely, and free of grammatical errors.
  • Work in partnership with children, families, caregivers, and other stakeholders to achieve permanency, safety, and well-being.
  • Coordinate and communicate regularly with Permanency Primers© and Hope & Healing team about court-ordered tasks and other activities required in meeting Conditions For Return and permanency goals (maintain and strengthen, reunification, permanency guardianship, adoption, independent living).
  • Assess, develop, and monitor child safety plans in accordance with Safety Methodology.
  • Develop, coordinate, and manage case plans that address underlying conditions that result in behavior change and conditions in the home that are supportive of reunification.
  • Responsible for monthly face to face contact with parent to check status of case plan compliance and working relationship between parent and Permanency Primer© and Hope & Healing.
  • Responsible for required face to face contacts with children at a frequency determined by risk.
  • Write fact-based, timely reports to the court that are free of grammatical or spelling errors.
  • Present fact-based information and recommendations to the court in a calm and concise manner.
  • Meet with supervisor and be prepared for scheduled supervisions.
  • Attend, participate, and be prepared for case staffings and huddles.
  • Attend all required trainings.
  • Participate and learn information presented during trainings to gain proficiency in all areas of responsibility.
  • Use tools provided to maximize the effectiveness of the Case Manager role.
  • Advocate and innovate for better ways of doing business.

Required Education / Training / Certifications

  • Bachelor’s degree in social work or related area of study or a master’s degree in social work or a related area of study from an accredited college or university. Related areas of study include bachelor’s or master’s degrees in human services, criminology, juvenile justice, psychology, sociology, counseling, special education, education, human development, child development, family development, marriage and family therapy, and nursing
  • Must hold a Child Welfare Certified Professional (CWCP) credential and uphold the FCB Code of Ethical and Professional Conduct
  • Experience : Minimum of 2 years post certification.
  • Required Skills

  • Strong verbal and written communication skills
  • Strong assessment skills related to risk and safety
  • Ability to learn and develop the skill of listening / observing and hearing / seeing the need behind the behavior when a stakeholder in their space is dysregulated.
  • Ability to ask for and accept help and support
  • Adaptive technical skills to navigate software that might be unfamiliar or used infrequently such as Teams, SharePoint, etc. as well as FSFN.

  • Physical and Mental Requirements of the Job :

  • Must be able to maintain a state of emotional regulation.
  • Engage in self-care strategies to build and maintain resiliency
  • Ability to access virtual or on-site locations.
  • Ability to operate a motor vehicle.
  • Must be able to tolerate some adverse environmental conditions when meeting with clients in the field.
  • Ability to stoop, kneel, crouch, and lifting when engaged with infants and young children.
  • Visual acuity to read, prepare, and analyze data, and characters on reports and on computer terminal, visual inspection of documents, and similar objects or items at a distance generally close to the eyes.
  • Ability to perform repetitive motions with wrists, hands, and fingers needed for computer work such as report writing, case documentation, etc.
  • Must be able to address and prioritize multiple task demands within established time frames.
  • Must be able to consistently meet deadlines.
  • Must be flexible in work hours to meet clients and other stakeholders as necessary.
  • As with all employees of 4Sisters, employee must pass extensive background screening required of our license and organization. These include Level 2 clearance in child welfare, local law enforcement, social media review, and Diana Screen® (www.dianascreen.com).
