A & B HomeCare Solutions, a Modivcare Personal Care Service, is looking for an LPN Nurse as a Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers/Certified Home Health Aides in a patient's home, evaluate the patient's plan of care and assess their physical and Psychosocial needs.

- Schedule: Monday through Friday, 8A-5p.
- Pay Rate range: $65K-$70K, depending on experience level.
- Visiting Client Territory: New Haven and surrounding areas

- Performs on-site supervisory and teaching visits to evaluate caregivers
- Provides care orientation to caregivers on assigned home care cases.
- Participates in the training of new nurses and in-services training for field staff.
- Performs initial nursing assessments
- Developing a plan of care and makes necessary revisions based upon patient's condition/needs.
- Document notes ensuring any changes in the service or patient's condition are reported
- Monitors records for complete and accurate documentation.
- Coordinates visits and communicate changes with the team
- Licensed Practical Nurse (LPN).
