Responsibilities :
The Patient Care Coordinator provides clinical support to A Better Homecare patients primarily through phone encounters. They answer inbound inquiries from patients, triaging and communicating to providers and staff with the appropriate urgency. They communicate clinical information to patients and staff for coordination of care. The Patient Care Coordinator is a member of the patient's care team, assisting with necessary care coordination efforts and proactively monitoring patients upon instruction.
