What You’ll Do:
As an LVN/RN Care Coordinator, you’ll support residents by:
- Coordinating care and ensuring compliance with care plans
- Communicating with families, providers, and the care team
- Monitoring resident health and wellness
- Helping maintain high standards for quality and documentation
Experience & Pay:
