Summary & Responsibilities
Summary: An Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness.
The ACT Team Leader is responsible for overseeing the administrative operations of the ACT team and providing clinical oversight of the services provided in conjunction with the Psychiatric Care Provider, as well as supervision.
Job Duties include:
- Supervise team members to assure the delivery of best and ethical practices
- Direct the day to day clinical operations including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours
- Lead daily organizational staff meetings and treatment planning meetings
- Direct and coordinate the client admission process and treatment rehabilitation, and support services of the program in coordination with the psychiatrist.
- Schedule admissions interview for new consumers, work with Assistant Team Lead to develop and coordinate initial assessment and initial treatment plan, assign the most appropriate staff to the individual treatment team and provide clinical supervision of the development of the comprehensive assessment and the treatment plan for each client
- Direct and coordinate the client centered comprehensive assessment of psychiatric history, mental status and diagnosis, physical health and dental health, use of drugs or alcohol, education and employment, social development and functioning, activities of daily living, and family structure
- Directly provide ACT services to an individual, where a therapeutic relationship is developed between the individual and the Team Leader
Basic Requirements:
- Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
- Valid driver’s license, auto insurance coverage and reliable transportation
- Pass criminal background check and other regulatory checks
- Provide proof of your education
Education and Experience Required:
- Master’s Degree in Special Education, Psychology, Social Work or closely related Human Services are
- Licensed clinician who has at (3) years three years of adult clinical experience with severe and persistent mental illness, with a minimum of 2 years post-graduate school.
- The Team Lead shall meet the requirements specified for licensed clinician, according to 10A NCAC 27G. 0104 (12).
- An associate level licensed clinician actively seeking licensure may serve as the Team Lead conditional upon being fully licensed within 30-calendar months from the effective date of hire. A Bachelor’s degree in Special Education, Psychology, Social Work or closely related Human Services area and 2 years of professional experience with the knowledge, skills, and abilities required by the population to be served.
- Exceptional written, oral, interpersonal, and organizational skills. High level proficiency in Microsoft Office- MS Word, Excel, Access, and PowerPoint. Or an equivalent combination of education and experience.
