Responsibilities
- Conduct comprehensive assessments to identify the needs of clients and develop individualized care plans.
- Coordinate services across various providers, ensuring seamless communication and collaboration.
- Provide crisis intervention and management support to clients facing immediate challenges.
- Mentor clients in developing coping strategies and life skills to enhance their quality of life.
- Supervise and support staff involved in client care, fostering a collaborative team environment.
- Advocate for clients within the healthcare system, including Medicare and other relevant programs.
- Facilitate workforce development initiatives aimed at empowering clients through education and job readiness training.
- Maintain accurate documentation of client interactions, progress, and outcomes in compliance with organizational policies.
Experience
- A degree in social work, psychology, or a related field is preferred.
- Experience in behavioral health, child welfare, or addiction counseling is highly desirable.
- Proven skills in crisis intervention and management are essential for success in this role.
- Previous mentoring or supervising experience is an asset.
- Familiarity with workforce development strategies will be beneficial for client empowerment initiatives.
