Your Role
The Pharmacy Coordinator, you will operationalize and lead a Pharmacy Program that is patient centric while fostering partnerships with our Primary Care Providers. In this role, you will work collaboratively with our Providers and Market teams to prioritize at risk patients, implement campaigns and conduct targeted patient outreach to ensure compliance of our patient population. You are accountable for the analysis of patient data, build necessary reporting capabilities and infrastructure to support continued growth and additional Markets. As the Pharmacy Coordinator, you will:
Execute a comprehensive Pharmacy program to improve chronic condition management, minimize disease progression and reduce medical expense
- Conduct patient outreach to assist patients with any medication barriers, assist with understanding medication regimen, refill schedules and insurance benefits
- Build strong relationships with our Market teams and communicate effectively the priorities and strategies
- Analyze and monitor patient data for our high-risk patients
- Utilization of payor partnerships, at-home delivery pharmacies and medication sync programs
- Conduct Provider and Market education on medication adherence and compliance. Develop training materials, resources and tools to support.
- Collaborate with our Patient Engagement team on patient outreach requirements and performance

Requirements:About You
- High School Diploma and 5 years working with pharmacy level engagements or support functions within Medicare Advantage/ Value-base care
- Bachelor's degree preferred
- Experience working Medicare Advantage Quality with an emphasis on Part D measures
- Retail pharmacy experience preferred
- Demonstrated ability to work independently, deliver results and drive compliance
- Strong communication skills and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders
