Part of training will be in Atlanta.

Community Resource Coordinator Responsibilities
Responsible for conducting home visits to high-risk members to complete a non-medical assessment to identify barriers impeding care such as home safety issues, transportation and other social determinants of health.
Connecting with community partners and / or organizations to build partnerships that help to address the needs of our members.
Coordinate, implement and participate in community events within the designated region but could include the entire Georgia market.
Community Resource Coordinator Requirements
Must have Reliable Transportation / Willingness to travel, travel will be 80% of the role. Travel will entail the entire state of GA.
Service Coordination Skills (knowledge of community resources and partners within designated region).
Strong communication skills such as verbal and written.
Strong organization skills and time management.
High School Diploma.
Microsoft Suite (PowerPoint, Word, Excel)
Valid Drivers License with proof of insurance.
