Field Case Manager - Social Worker
- WFH with up to 75% Travel Required – This is a remote position that requires in-person visits with members across the service delivery area.
- Service Area / Location : Fort Worth, TX (Western Hills, Benbrook, Wedgewood, Westover Hills)
- Pay Rate : $34.13 per hour
- Schedule : Monday - Friday, 8 : 00 AM - 5 : 00 PM (may require flexibility based on member preference)
