Job Description
The Resource Navigator seeks out new and shares current resources with the goal of supporting transition aged (15-24) young people. The Resource Navigator provides consultation to other professionals who are supporting transition age young people, as well as to provide brief resource connection to young people who need immediate assistance and are in the middle of accessing other services. The Resource Navigator works closely with multiple departments within New Day and specifically supports the Transitional Living Program through supporting admissions and new referrals and the Drop-In Center as a resource connector. The Resource Navigator represents New Day at community events to educate the community about New Day’s wealth of resources for transition aged young people.
Essential Duties
- Knowledge of youth systems with the expectation that employee will become expert in the adult system
- Meet as needed with youth, other team members and professionals to provide guidance, consultation, and expertise in bridging the youth and adult systems
- Facilitate resource navigation as needed for the transitional living team, wraparound team, and life skills coaches.
- Work as a team with other staff to ensure consistency, clarity and safety for young people
- Act as a liaison for New Day in the community with other programs that serve young people, probation officers, social workers, mental health professionals, businesses, health care providers and friends and family members of youth in the program.
- Enter all appropriate documentation both in on-line data base and independent spread sheets to ensure monthly reporting expectations are met.
- Keep consistent and detailed documentation on each young person per grant and programmatic expectations.
- Support young people in maneuvering through systems (SNAP, SSDI, TANF, etc)
- Provide services in a culturally competent and youth-friendly manner
- Work 1-2 days per week at New Day’s drop-in center location, holding regular "office hours" for youth to have contact and access to the navigator
- Manage transitional living program’s application process: initial meeting, wait list, set-up interviews and keep track of all documentation
- Participate on care coordination teams as needed to provide brief case management with an emphasis on resource connection.
Employment Requirements
- Must be 25 years of age or older
- Bachelor’s Degree and 2-3 years of experience working with youth, or Associate’s Degree and 3 years of experience
- Must have car, valid driver’s license, and maintain state required automobile insurance minimums.
