Are you passionate about coordinating care and supporting families in need? A New Hope (ANH) is looking for a dedicated Care Coordinator to help ensure our pediatric clients receive the highest quality private duty nursing care.
What You’ll Do:
- Act as the primary point of contact for families, nurses, and healthcare providers
- Coordinate care plans and ensure seamless support for our pediatric patients
- Schedule nursing visits and maintain accurate care-related documentation
- Work closely with clinical staff to address patient needs
- Monitor and track patient progress to ensure top-quality care
What We’re Looking For:
- Strong communication and organizational skills
- Experience in healthcare coordination, case management, or a similar role (pediatrics preferred)
- Ability to multitask and work in a fast-paced environment
- A compassionate, patient-centered approach
- Familiarity with healthcare software and systems is a plus
