A New Leaf is seeking a compassionate Case Manager to provide support and advocacy for survivors of domestic violence in a 24-hour emergency shelter. In this role, the Case Manager will conduct assessments, develop individualized safety and service plans, and connect clients to vital resources such as housing, legal assistance, counseling, and employment opportunities. You will play a critical role in empowering survivors to rebuild their lives, offering crisis intervention, emotional support, and guidance as they work towards independence and long-term safety. Knowledge of and / or experience with trauma informed practices and community resources required.

Hourly Pay Range : $20 - $24 (depending on years of experience)

Schedule : Sunday - Wednesday; 7am-5pm (four 10s)

Location : Glendale AZ

QUALIFICATIONS

Education & Experience :

  • Level I : HS Diploma / GED required & experience working with domestic violence, homeless or economically disadvantaged population required
  • Level II : HS Diploma / GED required & 1 year experience working with domestic violence population, in shelters, or other closely related human services experience; OR relevant higher education courses may be substituted for experience. 1 year experience working in case management preferred.
  • Level III :   HS Diploma / GED required & 3 years' experience working with domestic violence population, in shelters, or other closely related human services experience; OR relevant higher education courses may be substituted for experience. 1 year experience working in case management required.
  • Associate or Bachelor’s Degree, preferred.
  • Bilingual verbally in Spanish, preferred.

DUTIES AND RESPONSIBILITIES. Responsibilities will include but not limited to :

  • Role model appropriate behaviors to callers / clients in the shelter, at all times, utilizing trauma informed principles and practices.
  • Provide case management, crisis intervention, domestic violence education, and resources and referral services to callers on crisis calls and to clients in the shelter.
  • Communicate with hotel / motel and / or transportation agencies for client placement in overflow program and convey information appropriately to internal programs.
  • Be responsible for issues relating to admission and discharge, in conjunction with the programs definition and policies as relevant to program / client needs.
  • Perform screenings, lethality assessments, and basic needs assessments for potential clients according to procedure. Assist with the introduction of program and facility orientation to incoming clients. Assist with client intake / exit paperwork as needed.
  • Co-develop 24 / 7 Safety / Crisis Plans with every household, depending on the safety needs and risk factors identified by the client.
  • Assist households in obtaining community services and benefits, securing appointments with outside services and agencies as needed, including identification of funding sources and linkages to other agencies / services.
  • Provide referrals and / or coordinating transportation for households to community services as relevant to client need. Facilitate and coordinate discharge planning transition and follow up according to plan.
  • Recognize common symptoms of mental health related issues and / or substance use disorder, including common side effects, reactions / interactions of medications prescribed.
  • Maintain ability to drive agency passenger van to transport clients and complete other duties, as needed and / or drive personal vehicle to conduct program business (mail runs, grocery shopping, etc.).
  • Facilitate psychoeducational groups for clients based on topics relevant to client needs.

  • Class One Fingerprint Clearance Card
  • Valid Driver’s License with reliable transportation, required
  • Current Auto Liability Insurance, required
  • 39 Month Motor Vehicle Record, required
  • Be at least 21-years of age
  • Be able to lift a minimum of 30lbs
