A New Leaf is seeking a dedicated Case Manager to join our team at the East Valley Men’s Center, a shelter offering life-changing services to single men experiencing homelessness. In this role, you will provide critical support to residents as they work toward stability and independence. As a Case Manager, you will assess client needs, develop individualized service plans, and connect residents with essential resources such as employment, housing, and healthcare. You will also provide ongoing case management, track progress, and advocate for residents to help them overcome barriers to self-sufficiency.
This position requires a compassionate, resourceful professional with strong communication and organizational skills. Your work will directly contribute to fostering a safe, supportive environment that empowers residents to rebuild their lives.
Hourly Pay Rate: $23
Schedule: Monday – Friday; 8:00am – 4:30pm

QUALIFICATIONS:
Education:
High School Diploma or GED required.
Associate or bachelor’s degree in a related field, preferred.
Experience:
0 - 1 years of direct Case Manager Experience required
Experience working with individuals experiencing homelessness or those who are economically disadvantaged preferred.
Knowledge of community resources
DUTIES AND RESPONSIBLITIES: Responsibilities will include but are not limited to:
Work independently and collaboratively with teams, volunteers, and departments.
Assist a diverse clientele without bias, providing onboarding and immediate basic needs support.
Conduct client intake, assessments (e.g., SPDAT), and orientation training.
Provide case management, including developing and monitoring individualized service plans, tracking progress, and maintaining regular client contact.
Facilitate access to community resources, transportation, and external services as needed.
Document client interactions, demographics, and case notes accurately in HMIS and agency systems.
Coordinate discharge planning, ensure smooth transitions, and promote client self-sufficiency.
Educate clients on communal living, cultural awareness, and self-care.
Identify and address mental health and addiction concerns through referrals and resources.
Attend staff meetings, trainings, and Homeless Court hearings as required.
Provide client success stories and weekly progress reports to the program manager.
Adhere to ethical standards, program policies, and local, state, and federal regulations.
