A New Leaf’s Intensive Case Management Services H2O Program is dedicated to promoting housing stability and improving health and wellbeing for individuals served through AHCCCS. As a Tenancy Support Specialist, you will play a critical role in assisting participants with securing and maintaining stable housing.
This position involves working closely with individuals to develop personalized housing plans, connect them to resources, and provide ongoing support to ensure long-term housing success. By building trust and fostering self-sufficiency, the Tenancy Support Specialist contributes to increased stability and positive outcomes for participants.
Hourly Pay Rate: $25.00
Location: Maricopa County, AZ
Required New Employment Orientation located at HR Campus, Mesa AZ on the first week of employment; Monday - Friday 8:30AM-5:00PM
Office location for this position is based in Phoenix AZ
QUALIFICATIONS
Education:
High School Diploma/GED or equivalent required.
Experience:
1-2 years of direct experience, required
Experience working with various homeless populations, including families, individuals, young adults and/or homeless veterans.
Responsibilities will include but not limited to:
Support a caseload of 15 participants following H20 program guidelines, focusing on housing stability and resource connection.
Collaborate with participants' health homes, health plans, and housing partners for coordinated care.
Provide advocacy in locating affordable housing and assist with necessary documentation.
Conduct a minimum of four weekly check-ins with participants, per AHCCCS H20 requirements.
Develop and implement individualized Housing Care Plans to address barriers and promote stability.
Assist with independent living skills, employment, benefits acquisition, and other supports to maintain housing.
Help participants secure housing, set up utilities, and access essential items when needed.
Document case notes, referrals, assessments, and reports promptly using required systems (e.g., HMIS, Unite Us).
Ensure compliance with all training, agency, and AHCCCS program requirements.
Track and report caseload data and program outcomes to the Program Manager as needed.
Participate in meetings, trainings, and collaborative efforts with community stakeholders.
Promote a trauma-informed, culturally competent, and strength-based service environment.

Current Auto Liability Insurance
CLEAN 39 Month Motor Vehicle Record
Must Be At Least 21 Years of Age
