What are the responsibilities and job description for the Rapid Rehousing - Housing Case Manager I position at A New Leaf?
A New Leaf’s Rapid Rehousing program offers a unique solution to families and individuals across the Valley in need of help, providing utility, rent, and security deposit assistance. This program actively brings down the barriers to housing for those recovering from homelessness, reducing the months it would typically take to save for an apartment or home.
The assistance can be used for up to 12 months. The amount of assistance provided is based on case management assessments and verification of household income and debt. With temporary case management, support services, workforce assistance, and referrals to additional services, participants can address their challenges and maintain long-term housing stability.
Hourly Pay Rate: Starting from $19-$24 (depending on years of experience/education)
Schedule: Monday-Friday, 9am-5pm; flexibility needed

QUALIFICATIONS
Level I Case Manager:
High School Diploma or GED required, and 0-2 years of Direct Case Management experience preferred
Experience working with various homeless populations, including families, individuals, young adults, and/or homeless veterans highly preferred
Level II Case Manager:
High School Diploma or GED required plus 3-4 years of Direct Case Management experience OR Bachelor's degree in related field plus 1-2 years of experience direct Housing Case Manager experience

Level III Case Manager:
High School Diploma or GED required plus 4 years of Direct Case Management experience OR Bachelor's degree in related field plus 3-4 years of experience in direct Case Management experience

Primary Responsibilities:
Provide individualized support by helping each participant implement a Housing Stability Plan that addresses their barriers, increase their income, and maintain and sustain permanent housing.
Identify each area in which participants will need assistance to accomplish the outlined goals and objectives (i.e., scheduling appointments, applying for public benefits, etc.) and assist when possible.
Provide advocacy, case management, benefit establishment, linkage to Mental Health/Substance Abuse services, and other supportive services as needed.
Identify and coordinate with our housing partners, including HOM Inc., the documents required to conduct monthly evaluation of income to determine level of assistance.
Conducts SPDAT assessments, needs assessments, weekly and monthly home visits, check ins with landlords and/or property managers.
Use of personal vehicle to conduct home visits with the program participants.
Provides case management services including connecting participants with identification and other needed documents; assisting with obtaining food and furnishings; helping participants with credit repair and budgeting and utility information.
Provides services to participants in the areas of independent living skills, employment/job assistance, benefits acquisition, and other services as needed to assist in maintaining housing and fulfilling Housing Stability Plan goals.
Reply to participant requests for referrals and participant’s request for case management assessment and intervention within required response time.
Encourage and promote an environment that is strength based and culturally competent.
