A Physicians Home Care (APHC) is a locally owned business in Colorado Springs that was started by a family of a Special Needs child who is now an adult.
We founded in 2017 in hopes to be able to provide the best possible care to those who need in home assistance. Our motto is where everyone is treated as family and our main goals are to give back and help others. We have had the pleasure to help over 500 families and are licensed by the state of Colorado to provide In Home Support Services (IHSS) as well as Accredited by the state and ACHC to provide CNA, Skilled Nursing, and Therapy Services.

Pay: $21.00-$23.00 per hour
Schedule: Monday, Tuesday, Wednesday, Thursday and Friday. 8:30am-6:30pm or 8:00am-6:00pm. (flexible)

RESPONSIBILITIES:
· Provide services that are ordered in the plan of care and that the caregiver/aide is permitted to perform.
· Assist as assigned with activities of daily living such as bathing, grooming, toileting and elimination, and adequate nutritional intake, as well as preform transfers.
· Performs simple procedures as an extension of the therapy or nursing services as assigned. N-G tube.
· Understands and adheres to established Agency policies and procedures.
· Completes appropriate visit records in a timely manner as per Agency policy.
· Reports changes in the Client's condition and needs to the appropriate personnel.
· Performs household services essential to health care in the home as assigned.
· Attends in-service and continuing education programs as scheduled and necessary.
· Carries out, reports and documents care given in an effective, timely manner as observed by the agency and through periodic record reviews.
· Completes all work assigned by the Agency.
· Observes confidentiality and safeguards all Client related information.
· Reports any variance, accident, or unusual occurrence to the appropriate personnel.
· Maintains acceptable attendance status, per Agency policy.
· Appearance is always within Agency standard; is clean and well groomed.
· Demonstrates effective time management skills through showing up to shifts on time and daily documentation.
· Maintains clean and neat work environment.
· Demonstrates sound judgment and decision making.
· Performs other duties as assigned.
Qualifications:
· Must be able to lift up to a 75 lbs.
G-J-tube feeding experience
· Work positively in a team environment and favorably with patients, families, and staff
· 1 year of home health required
· Excellent interpersonal and verbal skills
· Computer skills and ability to operate and use agency’s clinical software program
· Strong interpersonal verbal and written communication skills
· Must be well organized and demonstrate the ability to prioritize job demand
