Job Description:
As a Home Care Coordinator at A-Plus Care, you will be responsible for overseeing the daily operations of our home care services. You will coordinate care plans, manage caregiver assignments, and serve as the primary point of contact for clients and their families. Your organizational skills and attention to detail will be crucial in maintaining the highest standards of care.
Key Responsibilities:
- Develop, implement, and manage individualized care plans for clients.
- Schedule and assign caregivers based on client needs and preferences.
- Communicate regularly with clients, families, and caregivers to ensure satisfaction and address any concerns.
- Monitor and evaluate the quality of care provided, making adjustments as necessary.
- Maintain accurate and up-to-date client records and documentation.
- Collaborate with healthcare professionals and community resources to enhance client care.
- Train and supervise caregivers to ensure adherence to A-Plus Care standards and protocols.
**Qualifications:**
- Previous experience in home care coordination or healthcare management is a plus.
- Strong organizational, communication, and interpersonal skills.
- Proficient in using electronic health record (EHR) systems and scheduling software.
- Knowledge of local healthcare regulations and best practices in home care.
- Valid driver’s license and reliable transportation.

Pay: $21.00 - $24.00 per hour
Schedule:
- Monday to Friday
- Rotating weekends
- Weekends as needed
