- Monitor patient health and report any changes or concerns to the appropriate healthcare professionals.
- Develop and implement individualized care plans tailored to each client's unique needs.
- Offer companionship and emotional support, fostering positive relationships with clients.
- Ensure compliance with resident rights and maintain confidentiality regarding personal information.
- Engage clients in memory care activities and cognitive stimulation exercises as needed.
- Provide assistance in home community care settings, promoting independence while ensuring safety.
Skills
