Job purpose
Under the direction of the Program Lead, the Patient Navigators (PN) are culturally competent non-clinical healthcare workers who support and guide patients throughout their care continuum. PNs support patients to anticipate, address, and overcome barriers to care while guiding patients through the delivery systems that provide their care. PNs work closely with the rest of the Health Center team to ensure high patient satisfaction.
Duties and responsibilities
- Engage potential patients and schedule new patient appointments.
- Conduct assessments and surveying patients to assess their needs.
- Perform warm handoffs.
- Enroll patients into eligible insurance programs; helping patients problem-solve insurance and billing issues; etc. Assist with screening and enrolling new ECM clients.
- Sign up patients for the Health Center patient portal.
- Provide care coordination for AACI clients.
- Provide translation and interpretation and non-technical explanations during medical visits and while making referrals.
- Document and maintain records per appropriate standards and regulations.
- Attend and participate in relevant departmental activities and meetings.
- Complete other duties and related projects as assigned.
Qualifications
Qualifications
- Ability to perform the duties described above.
- High School Diploma or Equivalent
- Two or more years of patient navigation or case management experience, or similar.
- Experience working in a health care setting.
- Aware of the needs of patients with serious health problems and limited resources.
- Committed to providing high quality customer service.
- Proficient with Microsoft Office applications
- Experience working with Electronic Health Records
- This position may require driving. A valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability policies.
- Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.
- Ability to communicate well with people of diverse cultural professional and experiential backgrounds and
- Language fluency verbal and written in English, and one of the following languages: Spanish, Vietnamese, Mandarin, Tagalog, Farsi highly desired.
- Patient Navigation Certification highly desired
Working conditions
Work is conducted in a standard community health center environment and in the community. Occasional evenings and weekend work is required.
