Requirements:
- MD or DO degree from an accredited medical school.
- Completed residency in Family or Internal Medicine accredited by the Accreditation Council for Graduate Medical Education
- Board certified or board eligible in Family or Internal Medicine
- Active and unrestricted medical license to practice in Arizona.
- Current DEA registration for prescribing controlled substances.
- Prior experience in a family medicine setting preferred.
