What are the responsibilities and job description for the Mental Health Case Manager PART TIME position at Abby Road Psychiatric Services?
Private psychiatric practice is seeking a case manager who will work in collaboration with our practitioners to coordinate services for mental health and addiction clients to promote wellness and recovery.
Responsibilities include:
Coordinate, schedule and assist in monitoring clients receiving in office medication treatment. Complete required documentation. Manage medication procurement and storage.
Provide navigation services for substance use disorder clients, receiving medication assisted treatment, to address psychosocial needs, recovery support, family education and referrals to alternate levels of care if needed.
Maintain continuous relationship with clients; proactively perform follow ups using validated rating scales to assess treatment adherence tolerability and clinical response. Routinely review results as required with treating practitioner.
Establish relationships with social services within the community and treatment centers to build a network for referral services. Maintain materials to assist clients in the referral process.
Assist clients in arranging services including transportation, housing, and other services as needed on an individual client basis.
Complete required documentation for clients including progress notes in collaboration with treating practitioner.
Requirements
Certified Medical Assistant with four years of related experience or Associate degree in related field with four years of related experience or Bachelor's degree in related field with years of related experience.
Part Time 24 hours weekly. Potential for full time in the future.
