We are seeking an experienced and detail-oriented Service Coordinator to oversee and manage homecare services for individuals in the Nursing Home Transition & Diversion (NHTD) program. - Coordinate and manage homecare services for NHTD clients, ensuring quality care and compliance.
- Serve as the primary point of contact for clients, families, and caregivers.
- Develop, implement, and monitor individualized service plans.
- Ensure compliance with state regulations and program policies.
- Communicate effectively with healthcare professionals, service providers, and agencies.
- Maintain accurate documentation and reports on client care and service coordination.
- Identify and address any service gaps to enhance client support.
- Prior experience in NHTD, TBI waiver programs, or homecare service coordination.
- Strong knowledge of NYS DOH regulations and Medicaid-funded programs.
- Excellent organizational and communication skills.
- Ability to work independently while coordinating with multiple stakeholders.
