Job Description:
Provides intermittent skilled nursing care to patients in the home.
1) Qualifications
- Licensed to practice Vocational Nursing by the California Board of Licensed Vocational Nursing and Psychiatric Technician Examiners.
- Minimum of one (1) year experience in nursing.
- Must be Spanish speaking and able to commute to Indio/Palm Springs area
2) Job Knowledge, Skills, and Abilities
- Ability to safely and effectively perform procedures and techniques involved in the delivery of patient care.
- Ability to follow a patient and family teaching plan.
- Ability to function as a member of the nursing team and cooperate with non-nursing professionals in the process of rendering nursing care.
- Ability to identify potentially harmful changes in patient condition and report these changes timely, verbally, and in writing.
- Ability to plan a persona; work schedule and work independently to complete the assignment on a timely basis.
- Ability to communicate effectively, verbally, and in writing.
3) Duties and Responsibilities Include
(but not limited to)
- Using the nursing process, in accordance with the Plan of Care (POC) or Plan of Treatment (POT): performs reasonable and necessary skilled care on each visit and documents this care in the record of the visit.
- Performs skilled observation of patient conditions.
- Contributes to the formulation and update of the nursing care plan.
- Performs complete bedside nursing when required.
- Assists the patient with activities of daily living and encourages appropriate self-care.
- Provides accurate and timely information on patient progressions in the care plan.
- Completes all required documentation accurately and timely.
- Attends In-Service presentations.
4) Physical Requirements
- Must be able to work as scheduled and report to work on time.
- Can stand or sit for up to six (6) hours per day.
- Able to lift up to twenty (20) pounds on a frequent basis.
- Able to push wheelchairs, turn patients, help patients walk, lift patients, and help patients sit up for four (4) to five (5) hours per day.
- Able to walk, stand, bend, climb, and crawl for four (4) to five (5) hours per day.
- Normal vision required, including depth perception, color perception, close inspection, and peripheral vision. Glasses or contact lenses may be used to insure normal (20-20) vision.
- Ability to hear normal speech, hear on the telephone, and hear faint sounds.
- Able to talk in person and on the telephone.
- Able to deal with stressful situations, including patient death, family grief, and trauma.
- Able to deal with patients, patient caregivers, and employees of residential care facilities, six (6) to eight (8) hours per day.
- Able to operate an automobile two (2) to four (4) hours per day.
5) Working Conditions
- Work generally performed in patients' homes or residential care facilities.
- Work requires employees to drive to where work is performed and employees will be outdoors, in or out of an automobile, one (1) to two (2) hours per day.

- Director of Patient Care Services (DoPCS)
- Work Remotely

Pay: $40.00 - $45.00 per hour
