**Job Description:**
We are currently seeking a dedicated and experienced LPN/LVN to join our team on a per diem basis. In this role, you will have the opportunity to provide skilled nursing care to patients in their homes, promoting wellness and independence while ensuring the highest standards of care delivery.
**Responsibilities:**
- Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the healthcare team.
- Administer medications, perform treatments, and provide wound care management as prescribed by the physician.
- Monitor patients' vital signs, symptoms, and response to treatment, reporting any changes or concerns to the appropriate healthcare professionals.
- Educate patients and their families about proper care techniques, medication management, and disease prevention strategies.
- Coordinate care with other members of the healthcare team, including registered nurses, therapists, and physicians, to ensure continuity of care.
- Maintain accurate and up-to-date patient records, documenting assessments, interventions, and outcomes according to regulatory standards and organizational policies.
**Qualifications:**
- Current LPN/LVN license
- Minimum of 2 years of experience in clinical nursing, preferably in home health or a related setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and prioritize tasks effectively.
- Valid driver's license and reliable transportation.
