We are seeking a compassionate House Calls Nurse Practitioner to join our team at Abode Care Partners.
About the Role
The successful candidate will provide personalized medical care to seniors and patients with complex needs in their homes or assisted living facilities.
Responsibilities
- Perform evidence-based medical assessments, including examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Collaborate with healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.
- Ensure patient medical records are kept up to date.
Requirements
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
