Nurse Practitioner - Home Health NP Role
The ideal candidate will provide medical care to seniors and individuals with complex needs in their homes or assisted living facilities.
Key Responsibilities:
- Deliver evidence-based medical care through comprehensive patient assessments and treatment plans
- Examine and treat patients with chronic and urgent episodic illnesses, minor injuries, and other conditions
- Order and interpret laboratory and diagnostic testing as necessary
- Collaborate with caregivers and healthcare professionals to provide interdisciplinary care
- Provide referrals to other healthcare practitioners when needed
Requirements:
- Active state Nurse Practitioner license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
