Job Description
As a Senior Medical Provider at Abode Care Partners, you will be responsible for providing medical care to seniors and patients with complex needs. Your duties will include evaluating medical needs, treating chronic and urgent illnesses, and collaborating with a healthcare team to ensure comprehensive care.
Key Responsibilities
- Evaluate and address patient medical needs using evidence-based practices.
- Treat chronic and urgent episodic illnesses, minor injuries, and other health issues.
- Collaborate with healthcare professionals to maintain a comprehensive plan of care.
- Communicate effectively with patients, families, and caregivers.
- Maintain accurate patient medical records.
Requirements
- Active state APRN license.
- Current National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of APRN experience preferred.
- Strong interpersonal communication skills – verbal and written.
- Excellent time management skills.
