Key Responsibilities :
Conduct comprehensive assessments to determine the care needs of patients.
Develop, implement, and evaluate individual patient care plans in coordination with caregivers and physicians.
Provide intermittent skilled nursing care to pediatric and adults patients at their residences.
Supervise paraprofessional staff and licensed practical nurses, ensuring compliance with Colorado State law.
Maintain accurate documentation and process orders, admissions, transfers, and discharge paperwork.
Ensure ongoing communication with all disciplines involved in patient care.
Adhere to federal, state, and agency policies and procedures.
Qualifications :
Graduate from an accredited school of nursing with an associate or bachelor’s degree in nursing.
Active RN license in the state of Colorado.
At least one year of nursing experience in home health care, with pediatric experience preferred.
Knowledge of state and federal home health agency regulations and compliance standards.
Current CPR certification from an American Heart approved course.
Valid driver’s license, reliable transportation, and active auto insurance.
Ability to perform physical assessments and provide care in non-wheelchair accessible homes.
Successful completion of a background check.
