Seeking a Community Case Manager in Lycoming County , Pennsylvania

Location : Lycoming County

Applicant must live in Lycoming County and be willing to travel through the county to service clients.

Abraxas Youth & Family Services is hiring a Community Case Manager to join our PATH Program (Partnering with Abraxas in The Home). PATH provides in-home services to youth and their families throughout Berks County, PA.

The position is responsible for assisting court committed youth and their families with the transition from residential placement back to the community or working with youth in the community who need additional support in effort to avoid out of home placement.

Salary : $23.00 per hour

Job Type : Full-time

Case Manager Basic Responsibilities :

Maintain a client caseload with appropriate service plans.

Develop and maintain relationships with community service sites. Develop and maintain relationships with assigned Juvenile Probation Department.

Orient the client to program norms, rules, and behavioral standards. Collect the YLS assessment from the Juvenile Probation Department and implement it into the service plan and the service delivery.

Build and maintain a positive relationship with the youth and their family through being caring, consistent and competent.

Support clients and their families in completing any court ordered conditions including restitution, community service, or specific counseling needs and assessment.

Supervise client activities and enters at least one note for each client into the Echo Advantage System bi-weekly.

Engage with clients prior to release from residential program to complete intake / interview process as needed.

Case Manager Hiring Requirements :

High school diploma and minimum of three years of related experience in the juvenile field; OR Bachelor’s degree in related field preferred and minimum of one year of related experience in the juvenile field; OR Master’s degree in related field and minimum of six months related experience in the juvenile field.

At least twenty-one (21) years of age

Driver's License

Physical exam that includes TB and drug testing.

Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.

Ability to work with computers and the necessary software typically used by the department.
