- Job Summary:
The Psychiatric Rehabilitation Coordinator is responsible for developing, implementing, and monitoring individualized rehabilitation plans for individuals with serious and persistent mental illness. This role focuses on promoting recovery, fostering independence, and helping clients build the skills necessary to live meaningful and self-directed lives in the community.
Key Responsibilities:
- Conduct comprehensive assessments of clients’ psychosocial needs, strengths, and goals.
- Collaborate with clients to develop individualized psychiatric rehabilitation plans (IRPs).
- Coordinate and facilitate group and individual rehabilitation interventions.
- Assist clients in building skills in areas such as self-care, social interaction, employment, housing, and education.
- Serve as a liaison between clients, families, and external providers to ensure continuity of care.
- Monitor client progress, revise rehabilitation plans as needed, and document services accurately and timely.
- Provide crisis intervention and support as necessary.
- Participate in multidisciplinary team meetings and contribute to holistic treatment planning.
- Ensure compliance with state, federal, and organizational regulations and standards.
- Maintain a recovery-oriented and trauma-informed approach to service delivery.
Qualifications:
Education:
- Bachelor’s degree in Psychology, Social Work, Rehabilitation Counseling, or a related field required.
- Master’s degree preferred.
Experience:
- Minimum of 2 years of experience in mental health, psychiatric rehabilitation, or a related field.
- Experience with community-based services and working with individuals with severe mental illness is highly desirable.
Certifications:
- CPR/First Aid certification may be required.
- Certification in Psychiatric Rehabilitation (e.g., CPRP) is a plus.
Skills and Competencies:
- Strong understanding of recovery principles and psychiatric rehabilitation practices.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively in a multidisciplinary team environment.
- Culturally competent and sensitive to diverse populations.

- Proficient in Microsoft Office and electronic health records (EHR) systems.
Working Conditions:
- This role may involve travel to client homes or community sites.
- May require evening or weekend hours based on client needs.
