Position Title: Psychiatric Rehabilitation Program Coordinator
Department: PRP
Reports To: PRP Specialist
Position Type: Part-Time, Contractor, hourly (15-20 hours/week)
Location: Germantown Office
Position Summary
The Psychiatric Rehabilitation Coordinator is responsible for overseeing and coordinating psychiatric rehabilitation services for individuals with mental health conditions. This position involves planning, implementing, and evaluating rehabilitative programs aimed at helping clients achieve greater independence and improve their overall quality of life. The Coordinator will work closely with clinical staff and their director supervisor, the PRP Specialist, to provide comprehensive care and support and connect clients with community services.
Essential Duties and Responsibilities
Program Coordination:
- Meet with assigned clients weekly or as necessary, based on the individual needs of each client and agency policy;
- Perform rehabilitation activities at the office, in clients’ homes, and in the community;
- Develop, plan and coordinate individualized psychiatric rehabilitation programs tailored to the needs of clients with mental health disorders.
- Implement culturally appropriate intervention strategies including goal setting, planning, and decision making.
- Ensure that all programs comply with state and federal regulations, organizational policies, and best practices.
- Collaborate with interdisciplinary teams to ensure holistic service delivery.
Client Support:
- Conduct assessments to determine client needs, preferences, and goals related to psychiatric rehabilitation.
- Provide direct support and guidance to clients through one-on-one or group rehabilitative interventions.
- Monitor client progress and make necessary adjustments to their rehabilitation plans to ensure optimal outcomes.
- Maintain consistent contact with client and work cooperatively with other members of the client’s treatment team;
- Uphold with high regard, the confidentiality of clients and follow all HIPPA guidelines.
Documentation and Reporting:
- Timely completion of documentation and progress notes within 24 hours of visit.
- Complete and submit thorough, accurate documentation including in encounter notes, Individual Rehabilitation Plans, discharge/transition recommendations and summaries; in accordance with agency policy and all applicable local, state, and federal regulation.
- Maintain accurate and up-to-date client records in compliance with legal, ethical, and organizational standards.
- Prepare reports on client progress, program outcomes, and operational efficiency for management review.
- Monitor and report on key performance indicators to assess the effectiveness of rehabilitation programs.
Community Liaison:
- Build and maintain relationships with external service providers, community organizations, and support agencies to ensure comprehensive care for clients.
- Advocate for clients' needs and rights within the community and ensure they have access to available resources.
Compliance and Quality Assurance:
- Ensure all programs meet the established guidelines for psychiatric rehabilitation, maintaining a high standard of care.
- Participate in audits, inspections, and quality improvement initiatives to ensure continuous service improvement.
- Stay informed on trends in mental health and rehabilitation services, ensuring that the organization remains up-to-date on evidence-based practices.
Required Qualifications
- Valid Driver's License
- Reliable mode of transportation
- Knowledge about and ability to provide skills training
- Minimum requirement: A Bachelor's degree in social work, counseling, or related field or at least 2 years experience in the mental health field.
- CPR or first aid certified (or willing to obtain within 6 months of employment).
Preferred Qualifications
- Relevant certifications (e.g., Certified Psychiatric Rehabilitation Practitioner (CPRP)) are preferred.
- Strong Desire to work with clients who have severe mental health conditions.
- Knowledge of health information related to adult health and mental health, including co-occurring mental health and intellectual disability conditions of individuals.
- Work as a team member with co-workers and agency staff.
- Cultural sensitivity and understanding of clients and individuals with co-occurring intellectual disability and mental disorders.
- Commitment to maintaining networking relationships with community service providers.
- Experience working with diverse populations, including trauma, substance use, or severe mental illness.
- Proficiency in maintaining documentation and working with electronic systems.
- Bilingual abilities.
Working Conditions
- Combination of office, community-based and occasional residential visits.
- Regular client-facing interactions and occasional evening sessions, as needed.
- Must comply with safety protocols and maintain a professional therapeutic environment.
Physical and Technical Requirements
- Ability to sit for extended periods during sessions.
- Occasional travel may be required to visit clients in the community or to attend training sessions and meetings.
- Proficient in using a computer and EHR software and authorizations
- Ability to respond effectively to crisis situations, as needed.
