Licensed And/or Certified Pharmacy Technician Required
Work Hours 900 a.m. - 600 p.m.
Position Purpose Collaborate with the pharmacists to coordinate patient care for patients receiving infusion therapy

Position Purpose Collaborate with the pharmacists to coordinate patient care for patients receiving infusion therapy
- Coordinate the treatment process from initiation to the conclusion or discontinuation of therapy
- Under direct supervision of a pharmacist performs prescription data entry functions
- Coordinates home nursing services
- Assists and maintains all related pharmacy records (prescription logs, care plans, medication profiles, and any other related files) as determined by the pharmacist
- Obtain verbal authorizations and request detailed clinical information from prescribers
- Track and triage coverage determination requests submitted from providers and determine if a pharmacist review is required
- Approve coverage determination requests based on defined criteria
- Respond to client inquiries regarding authorization approvals and PBM on-line applications
