JOB SUMMARY:
The Social Services Assistant works under the direction of the Social Services Manager and Admissions Coordinator to perform clerical duties necessary for the smooth admission and discharge of residents from ACC Care Center. This position is a non-exempt (hourly) position. This position reports to the Social Services Manager with a matrix reporting responsibility to the Admissions Coordinator.
ESSENTIAL JOB FUNCTIONS:
- Greets & tours admission candidates; welcomes new residents and orients them upon arrival.
- Ensures rooms are prepared for new admissions; posts names and prepares wheelchair nameplates.
- Assists with obtaining and tracking insurance authorizations as needed, setting up short and long stay Care Conferences and family meetings.
- Develops rapport with short and long stay residents; resolves issues to help facilitate an excellent experience while within the care of ACC Care Center. Administers discharge survey to short stay residents prior to discharge.
- Reviews discharge summary from the hospital for short stay residents to ensure appointments and referral needs are addressed timely.
- Follows up on home health referrals and documents in PointClickCare (PCC).
- Assists with setting up post-discharge needs and services for discharging residents including medical equipment, follow-up appointments, primary care physician appointments, home health, transportation, etc.
- Sends updated discharge emails to the interdisciplinary team.
- Perform a variety of clerical duties as assigned by the Social Services Manager.
Qualifications
QUALIFICATIONS:
- High School graduate or equivalent.
- Two years of Skilled Nursing Facility experience preferred.
- Effective English communication skills, both verbal and written.
- Bilingual a plus!
- Excellent organizational skills.
