Essential Functions:

• Complete accurate and timely MDS assessments in accordance with regulatory requirements.
• Develop and update care plans based on MDS findings, ensuring the best outcomes for residents.
• Collaborate with the healthcare team to ensure continuity of care and compliance with federal and state regulations.
• Monitor and report changes in residents' conditions.
• Assist with audits and quality improvement initiatives related to MDS processes.
  
  
  

What We’re Looking For:

• Must be a licensed LPN or RN with experience in long-term care or skilled nursing.
• Knowledge of MDS process and regulatory compliance.
• Excellent organizational, communication, and interpersonal skills.
• Ability to work collaboratively with interdisciplinary teams.
