Now Hiring: MDS Coordinator
Essential Functions:
- Complete accurate and timely MDS assessments in accordance with regulatory requirements.
- Develop and update care plans based on MDS findings, ensuring the best outcomes for residents.
- Collaborate with the healthcare team to ensure continuity of care and compliance with federal and state regulations.
- Monitor and report changes in residents' conditions.
- Assist with audits and quality improvement initiatives related to MDS processes.
What We’re Looking For:
- Must be a licensed LPN or RN with experience in long-term care or skilled nursing.
- Knowledge of MDS process and regulatory compliance.
- Excellent organizational, communication, and interpersonal skills.
- Ability to work collaboratively with interdisciplinary teams.
