JOB SUMMARY: In support of the Access Mission and Strategic Commitments, the Community Health Worker - Resident Service Support provides support to residents through a combination of information and referral to services, coordination of on-site programming, and addresses social determinants of health-related services provided. (Community Health Worker training provided as part of the hiring process)
ESSENTIAL FUNCTIONS and DUTIES:
- Provide support to residents through a combination of information and referral to services
- Conduct assessments to determine resident needs and develop service plans, if necessary
- Provide ongoing outreach services to identify individuals who would benefit from assistance, recruit volunteers, and determine program and service gaps
- Maintain all necessary information regarding services to residents
- Oversee and provide crisis intervention, case management, and follow-up services to referrals from management, residents, or other agencies
- Identify funding for new and expanded programs in the development
- Review and submit all billing and program reports required by funding sources, monitoring entities, and Access
- Coordination of on-site programming
- Support resident efforts in community-building initiatives
- Develop and implement supportive service programming with residents, management, and local community service providers
- Act as a liaison to local social service providers and coordinate the delivery of programs/services from them
- Address social determinants of health related to services provided
- Provide services within the program services that will have the potential to improve customer health outcomes
- Other duties as assigned
- Participation and completion of training requirements related to Community Health Worker Certification

QUALIFICATIONS:
- Bachelor’s degree in a related field plus a minimum of three (3) years of experience or a Master’s degree in a related field and a minimum of one (1) year of experience*
- Effectively works independently in a fast-paced environment
- Works well with a diverse group of residents and staff
- Problem solver, solution generator
- Understands and utilizes basic budgeting principles and skills
- Good computer skills with working knowledge of word processing and spreadsheet applications
- Bilingual in Spanish and English preferred
- Complete Community Health Worker Training within four months of training availability

PHYSICAL DEMANDS:
- Light Work - Lifting no more than 20 pounds occasionally and up to 10 lbs frequently
- Frequently required to continuously stand, walk, sit, and consistently use either hand or foot controls
- The person in this position often communicates with individuals outside the organization
- Must be able to hear and vocalize effectively to exchange accurate information
- This position may require the ability to transport self off-site into environments the agency can not control
- The employee will occasionally be required to read and comprehend complex grant documents, program plans, and agency policies and procedures
