Position Summary
The grant-funded Consortium Peer Advocate, Maternal & Child Health (MCH) serves as the co-chair of the Westside Healthy Start Community Consortium and carries a small caseload of Westside Healthy Start patients. The Consortium Peer Advocate, MCH will assist patients in accessing maternal health services and support patient empowerment through health education and coaching. The Consortium Peer Advocate, MCH will assist Program Coordinator, MCH in the development and execution of the Westside Healthy Start Community Consortium.
Core Job Responsibilities
Assist Program Coordinator, MCH in the development, execution, and documentation of an annual action plan that addresses barriers to care, improves the local perinatal care system, and addresses social determinates of health.
Assist in the planning and execution of, and attend, all Westside Healthy Start Consortium meetings and events.
Participate in national Healthy Start training opportunities and events.
Provide case management services in accordance with the Westside Healthy Start curriculum to patients that have been assigned or referred to Westside Healthy Start.
Provide ongoing follow-up with patients and/or service providers to determine whether patients have accessed services. Follow-up should be continuous from initial identification through case closure.
Serve as part of the patient’s care team and support health center operations and attainment of organizational metrics.
Establish effective and respectful relationships with patients, clinic staff, and community organizations and stakeholders.
Assist in developing/maintaining community referral relationships through community outreach and effectively connecting patients and families to community resources. This can include, but is not limited to, hosting a table at community events, attending community partner meetings, and building relationships in the local community.
Engage in community planning groups and/or meetings to support the local perinatal health system.
Complete documentation and data entry as needed to assure optimal patient care and program reporting.
