Job Summary:
We are seeking a highly skilled VA Pharmacist to join our healthcare team. The pharmacist will be responsible for ensuring the appropriate use, storage, preservation, and distribution of medications to enhance patient care. This role requires expertise in medication management, patient counseling, and collaboration with healthcare professionals to ensure optimal therapeutic outcomes.
Key Responsibilities:
- Dispense medications and provide pharmaceutical care to veterans in accordance with federal and state regulations.
- Ensure the safe and effective use of medications by reviewing prescriptions for accuracy, appropriate dosing, and potential drug interactions.
- Counsel patients on medication usage, side effects, storage, and adherence to prescribed therapies.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care plans.
- Monitor patient responses to prescribed medications and adjust therapy as needed.
- Maintain accurate pharmacy records and comply with VA regulations and policies.
- Educate patients and healthcare staff on new medications, therapies, and best practices in pharmaceutical care.
Qualifications & Requirements:
- Licensure: Applicants must hold a valid license to practice pharmacy in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a U.S. territory.
- Education: A Doctor of Pharmacy (Pharm.D.) degree from an Accreditation Council for Pharmacy Education (ACPE) accredited program or an equivalent accredited body recognized by the U.S. Department of Education at the time the degree was obtained.
- Experience: Previous inpatient pharmacy experience is preferred.
- Strong knowledge of drug therapy, pharmacokinetics, and medication safety.
- Excellent communication and interpersonal skills to work effectively with patients and healthcare teams.
- Proficiency in electronic health record (EHR) systems and VA-specific pharmacy technologies is a plus.
This position is contingent upon contract award, client approval, successful background investigation, and meeting credentialing requirements.
