Job Description :
We are seeking a skilled and experienced Home Infusion Pharmacist for a 13-week contract-to-hire position in Phoenix, AZ. This role is focused on working with patients who require infusion therapy for acute conditions. The Home Infusion Pharmacist will be responsible for preparing sterile pounds, including IVs, hydration solutions, TPNs, and antibiotics. This is a fantastic opportunity for a pharmacist with a background in infusion and clinical pharmacy to make a direct impact on patient care.
Key Responsibilities :
- Prepare and dispense sterile pounds for infusion therapy, including IVs, hydration, TPNs, and antibiotics.
- Provide clinical expertise and guidance to patients, caregivers, and healthcare providers.
- Monitor patient medication therapy and ensure the safe administration of infusions.
- Assist with patient education on proper medication use and potential side effects.
- Collaborate with the healthcare team to ensure optimal patient outes.
- Participate in rotating "on-call" weekend shifts to support 24 / 7 care.
