INDEPENDENT LIVING SPECIALIST / INTAKE
Job Summary: Under direction of the Program Manager, coordinates the referral and intake process for Access II Independent Living Center; performs related duties as assigned.
DIRECTLY RESPONSIBLE TO
Program Manager
DUTIES AND RESPONSIBILITIES (Any one position may not include all of the listed duties, nor do all of the listed examples include all tasks which may be found in positions within this classification.)
Coordinates the referral and intake process for Access II Independent Living Center; acts as a resource to families, outside agencies and staff; answers telephones; enters and maintains referral/intake information using a database; maintains contact with consumers awaiting approval for services; processes referrals including preparing intake and initial assessment packets for ILS team; maintains and update resources in Referral Contacts in CIL Suite; assist with other IL tasks as needed; provides program support for team/staff as needed; maintains cooperative working relationships with staff, families and agency personnel.
Oversee the Durable Medical Equipment and Transportation Loan Programs; coordinate the maintenance of the Center vehicles. Additional clerical work as assigned.
Documentation & Document Management: Document goals and services provided; maintain clear and concise records including notes of all contacts; file paperwork related to the program; compile activity/program reports as requested; maintain confidentiality of consumer records.
Training: Attend staff meetings and participate in staff training opportunities.
Other Duties: Maintain positive close working relationship with all service agencies. Conduct marketing and outreach (informational presentations to community based organizations and outside agencies regarding Access II Independent Living Center programs) as time and opportunity allows. Perform other duties as assigned.
Qualifications:
A.)The ideal candidate will hold a bachelors degree in a Human Services related field or have at least 2
years experience in providing services to persons with disabilities.
B.) This position requires excellent written and verbal communication skills, organizational aptitude, and
attention to detail.
C.) Must be highly organized, self motivated and have ability to multi task and meet deadlines.
D.) Prefer personal experience with disability.
E.) Understanding of and adherence to the philosophy of independent living and the disability rights movement such as the ADA, Civil Rights Act of 1964, Fair Housing, and Transportation Act.
F.) Knowledge of community, state & national resources.
