Salary : $20 per hour DOE
The HCV Patient Navigator will assist the team with program implementation. They will help patients at risk, secure rapid and confirmatory testing, provide linkage to care, help with medication adherence and retention in services, facilitate testing for HIV / HCV, assist as part of a team to deliver Hep C and HIV test results to clients, conduct patient follow-up, and participate in Hep C Treatment, HIV Treatment and PrEP promotional campaigns, outreach, data entry and other duties, as needed.
The HCV patient navigator will take the lead with those testing negative to ensure the patient is educated about safe behaviors and PrEP, receives the necessary lab work to start PrEP, and is assisted with the costs of medication so as not be a barrier to adherence and patient care.
Responsibilities :
Testing & Prevention Services 75%
- Successfully Completes HIV Test Counselor Training
Provides confidential assessment to determine the HIV / HCV prevention needs of the clients whether test results are positive or negative; this includes the registration and planning of prevention services;
Provides access to prevention services, ensuring access, follow-up, and adherence to treatment, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance;
Collaborates with project staff in advertising and media promotion of the Hep C and HIV testing programs;
Provides community outreach and education
Provides case management services to PrEP clients
Attends SSP and outreach events, conducts harm reduction services
Other duties 25%
Participates in ASN fundraising
Attends and participates in weekly staff meetings
Performs all other duties, as assigned and necessary for ASN operations.
